Please ensure you consider the following points when making a referral:

• has the young person been consulted?
• does the young person understand the consent form and has he/she signed it?
• have other relevant key agencies/parents/carers been consulted?
• does the young person already have a Personal Adviser with whom you can liaise with?
• have the NEET procedures been followed where necessary?
• has the referral form been completed?
• has the referral been accepted and has an appointment been made?
• have arrangements been made to accompany the young person to this appointment?
• have there been discussions on follow-up/review and any joint-working arrangements?
• if there are difficulties with accessing Personal Adviser services or if you are unsure of which agency to refer the young person to, has contact been made with the Connexions local manager?
