The holder of a premises licence must notify the licensing authority of any change in their name or address.
Only the premises licence holder can apply for a copy of the licence or a change of name/address using the appropriate form.
Where the designated premises supervisor is not the holder of the licence, they may also notify the licensing authority of any changes in their name or address - and then give a copy of this notification to the holder of the premises licence.
